Non-Covered Services:
At times there are services desired by patients which are not available as covered services under most or all insurance plans. These services may be purchased, but will be billed only to the patient and not to insurance. These services are only available if and when they do not place the patient at unreasonable medical risk in Dr. Skaggs’ best medical judgment.
Telephone Management, New Problem, Low Complexity
One or two prescriptions $20.00
Telephone Management, New Problem, Medium Complexity
Three or more prescriptions $30.00
Extension of Prescription for recent acute problem $20.00
(not available for controlled substances)
30 Day Bridging Refill or Replacement Prescription, 1-3 $15.00
30 Day Bridging Refill or Replacement Prescriptions, 4-6 $20.00
30 Day Bridging Refill or Replacement Prescriptions, 7-10 $25.00
30 Day Bridging Refill or Replacement Prescriptions,
each additional over 10 prescriptions $3.00 each
Note: The need for bridging refills can be completely avoided by scheduling an appointment before existing prescriptions run out.
Phone Counseling:
10-20 minutes $40.00
21-30 minutes $50.00
31-40 minutes $60.00
41-50 minutes $70.00
51-60 minutes $80.00
A charge for phone counseling will not be applied for answering questions about recent prescriptions.
Forms & Letters:
Simple $10
Moderate $20
Complex $30
There is no fee for completion of forms during a visit. Please note that the standard fees for form completion in the Lexington market starts at $60 and goes up from there.
