Payment Policies
Answers to common questions
Copays must be paid at the time of visit. An additional charge of $5 will be assessed if the copay is not paid at the time of the visit.
Coinsurance is similar to a copay, but is usually a percentage of the “allowed amount,” so we only know how much it is after your insurance processes the claim. We must then bill you for the coinsurance amount. Many policies require you to pay both a copay AND coinsurance.
Missed appointment fee of $25 is assessed for failure to keep appointments without 24 hours advanced notice. This fee may be waived in cases of verified emergency.
Uninsured patients are welcome. We make every effort to keep costs affordable. We will try to estimate the cost of your visit ahead of time; be sure to tell us if you are paying out of pocket.
Non-covered services are services for which you must pay out of pocket because insurance excludes them from your benefits. In the past, this was mostly limited to things like cosmetic surgery, but as insurance plans seek to maximize their profits, many more services are being left non-covered. Examples of commonly excluded services include: Preventive Examinations (complete physicals), services related to obesity or weight loss, services related to mental health, immunizations, medications provided in the office, services or medications for sexual problems, ear wax removal, Administrative Exams (school or sports physicals, adoption physicals, Commercial Driver’s physicals, work-related physicals. If we know or have strong reason to believe that a service you wish to receive is non-covered, we will tell you ahead of time, and you may pay a discounted fee at the time of service. If we only learn later that we have provided a service for which you had no insurance coverage, you will be billed the full fee for that service.
Worker’s Comp is not accepted at this office. If you have a work related injury check with your employer to see if they contract with a particular provider for those services. If they don’t, you should seek treatment at an urgent treatment facility, emergency room, chiropractor or other office that will file a Worker’s Comp claim for you.
